Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records
  • Completed patient forms

Appointments and Cancellations

We make every effort to schedule you at your convenience, and we see acutely ill patients as soon as possible. Routine physicals need to be scheduled in advance. Please call and cancel any appointment you cannot keep so we can provide that time for other ill patients. We do charge for your second "no-show" visits at a rate of $85 billed directly to you. A third "no show" may result in termination of the provider-patient relationship. We charge for all office visits, even follow up appointments and blood work, unless other arrangements are made with your provider.

Referrals

Referrals are done on an as-needed basis, with patients needing to be seen by a provider prior to any referral for a new problem being done. Turn around time for referrals is 7 to 10 working days.

Hospitalization

Our providers use a hospital-based group of Internists/Hospital Specialists who can provide prompt, high-quality care for our hospitalized patients. Once your hospital stay is over, the Inpatient Team turns your care back over to us after your discharge.
