Insurance Information
We accept all major HMO and PPO insurances, PHCS, Health Savings Accounts, and self pay patients. We do not accept Medicaid, Medicare, Kaiser HMO. We do not honor medical discount cards. For insurance questions, call or e-mail our billing department.
If you have insurance, please bring your insurance card with you to your appointment, along with your copayment, which must be paid at the time of service. We reserve the right to refuse care if copayment cannot be paid at the time of your visit. If we are listed as providers for your insurance company, we will submit your claims for you. If you have no insurance, we request payment at time of service. We accept cash, check, Master Card, Visa, and Discover. There is a $25 charge for all returned checks. We are always happy to discuss your bill and make arrangements with you for payment if necessary. Any bills over 60 days are subject to a monthly rebilling charge. Accounts that are sent to collections are grounds for patient dismissal. If you have any questions or problems with your bill, please contact our bookkeepers at 303.238.9415.
